Connected Care Systems Puts ConnectedCare-dx Into Test Markets
A sensor mesh and an AI adherence engine turn a home into a monitored environment — no cameras, no microphones, nothing to remember to wear.
MDC Studio portfolio company Connected Care Systems has its home monitoring platform, presented as CARE ElderHome and served to clinicians and families through connectedcare-dx.com, deployed with early home-health customers and running pilots in several markets. The system unifies motion, environmental, and wearable inputs into a single stream describing what is happening in a home, and applies an adherence engine that learns each resident's baseline before flagging what departs from it.
Around 90% of adults over 65 would prefer to stay in their own homes, but until now there has been little visibility into safety, medication adherence, or day-to-day status outside a clinical setting. Falls are the leading cause of injury death in that age group and most go undetected for hours. Medication non-adherence drives roughly one in ten hospital admissions. Home-health agencies visit periodically, which leaves more than 22 hours of each day unobserved, and consumer wearables do not connect to clinical workflow or to billing.
ConnectedCare-dx is built to sit inside existing clinical practice rather than beside it. The backend is Python and Flask with FHIR R4 clinical integration and Twilio-based alerting, so a deviation is routed into the workflow a provider already uses instead of into another separate application. Clinicians and family members see different dashboards, and the platform can be deployed as a standalone service or embedded into a home-health or assisted-living operator's own offering.
The company is aligning with the CMS remote therapeutic monitoring codes 98975, 98976, and 98977, which make each monitored patient reimbursable on a per-patient-per-month basis and give home-health agencies a billing route for continuous monitoring. Anchor channels are home-health agencies and Medicare Advantage plans, with extension planned into visiting-nurse and assisted-living operators and into post-surgical monitoring.
Connected Care Systems has an application pending with the National Institute on Aging for a $1 million SBIR contract.
